This receipt is from Canada (note the GST column). No province in Canada has a minimum wage that low, not even for servers.

That column is dollars per guest. Divide any value in the AMT column by the total number of guests, 638, and you get the $/GST.

The college I work at has faulty change maker in the vending machine room, I've found a few dollars randomly spat out.

This can’t be in the US! Servers in my state only get paid 2.50 an hour.

It could be average wage including managers, chef, etc or this could be the combined tip/wage situation. Where they are factoring tip into the wage.

Not in Cali. There's no such thing as tipped minimum wage. All workers must be paid state minimum regardless.
